
How to refer a patient

On the outgoing referral page, search/select for “SimpliFed” provider in the “To” provider field and select “Lactation Specialist” under Specialty.

In the “Reason” field, type in a reason via free text such as "lactation or baby feeding consultation. Also check that the patient’s email is in the demographic tab for us to receive the email to send the secure video link.
In the “Diagnosis” field, type in "Z3A.30" or any other relevant diagnosis code.
Hit send referral. When you choose the referral, include “Attach CCR/CCD” and “Attach Medical Summary” (please check both).
Recommended Referral Timing:
Many practices choose to automate or standardize referrals to SimpliFed around 28 weeks gestation to proactively support families before delivery and throughout postpartum recovery.
What Happens After Referral?
Once a referral is received:
1. SimpliFed contacts the patient directly.
2. Patients are scheduled virtually with our maternal health specialists.
3. Care is coordinated with the referring provider when appropriate.
4. Families receive ongoing support across feeding, newborn care, and postpartum transitions.
